Eye bags are a common reason for consultation, both for women and men, and are due to prolapse of fat in the lower eyelids. As a result, the area under the eye appears swollen or puffy, giving the eyes a tired or aged appearance.

There are two possible treatments for bags. One is to remove them through surgery; the other is to hide them with hyaluronic acid.

Surgery allows definitive treatment. The aim is to eliminate these pockets of fat conservatively or reposition them. It can be made through an incision that is hidden in a fold just below the eyelashes, or also through the conjunctiva where the scar is completely hidden. The choice between these two will depend on whether excess skin needs to be dried or not.

It is performed in the operating room, with sedation and is an outpatient procedure. It’s not painful. The care is to avoid exertion and the sun to avoid inflammation of the eyelids. It is generally associated with surgery of the upper eyelids to eliminate excess skin and thus clear the eyes, preserving the natural features of each patient. Finally, as a complement, the patient’s own filtered fat can be injected into the cheekbone area to improve the shine and texture of the skin and a CO2 laser to tighten.

On the other hand, there is the hyaluronic acid filler. A special filling is used for this area, which does not swell too much. The objective is to camouflage the bags, improving the appearance of the tear valley. It is performed in the office, using a needle or cannulas. The discomfort is usually minimal. The result usually lasts a year. The complications of this treatment are hematoma or the filling being very superficial.

An important concept is that not all patients are candidates for filler, since sometimes it can be counterproductive and swell the bags more, and in this case the filler with hyaluronidase must be eliminated. To improve the color of dark circles, peels or depigmenting creams are usually performed.
