Titan disaster “could have been prevented”: “Crucial inspections, analyzes and preventive maintenance completely ignored”

The American Coast Guard concludes that the imploding of submarine Titan is mainly due to the non-follow-up of safety, maintenance and test protocols by the OcEngate company involved. The five occupants died when they were on their way two years ago to the Titanic in the North Atlantic Ocean.
