Damning new report on Titan disaster: negligence and corporate culture at the root of implosion

Three years after the Titan submarine’s fatal implosion, Canadian safety officials have released a new report on its disastrous final voyage. They conclude that the American company behind the expedition was guided by “confirmation bias” and largely ignored the risks.
