Researchers have identified a previously unknown cell death process in the brain. This so-called karyoptosis could be the key to new therapies against Alzheimer’s and frontotemporal dementia (FTD).

Understanding Cell Death Mechanisms

A team from King’s College London and the UK Dementia Research Institute describes the mechanism in the journal Nature Communications. The trigger is the accumulation of toxic proteins in the brain. The enzyme p38 MAP kinase then interacts with the protein LaminB1, leading to the collapse of the cell nucleus.

The numbers speak for themselves: In the frontal cortex of Alzheimer’s patients, 35% of cells showed karyoptosis. In healthy individuals, the figure was only 15%. In laboratory settings, researchers have already succeeded in halting the process: Blocking the p38-LaminB1 interaction led to a decrease in markers of cell death.

Enhancing Early Detection

Simultaneously, researchers are working on early detection methods. A study from Ruhr University Bochum, published in EMBO Molecular Medicine, examined blood biomarkers in 779 subjects over 17 years. The result revealed that the misfolding of the amyloid-beta protein is the strongest predictor of future disease during symptom-free stages (AUC value: 0.79).

Another often-quoted biomarker, P-tau217, lagged behind with a score of 0.67. Only a marker panel achieved a predictive accuracy of 0.87. Additionally, a study in Science on July 24 showed that the 3D structure of the genome changes in Alzheimer’s patients, parallel to the decline of synaptic programs.
