Ocrelizumab: A Ray of Hope for Advanced MS Sufferers
People often believe that nothing can be done for advanced cases of Multiple Sclerosis (MS), especially when faced with progressive forms of the disease. However, a recently published study brings new hope, revealing that even patients previously labeled as hard to treat may benefit from an existing medication—Ocrelizumab.
The Evolution of MS Treatment Options
Over the past decades, therapies for MS have evolved remarkably. Attacks, known as relapses, can now be effectively suppressed in most patients. However, a significant challenge remains: the progression of disability independent of these relapses. Chronic progressive forms of Multiple Sclerosis often do not respond well to currently available medications, leaving many patients with limited options.
Understanding Primary Progressive MS
Ocrelizumab was further investigated in a Phase IIIb study. The addition of the “b” signifies that Ocrelizumab is already an approved treatment, but this study targeted a patient population for which efficacy had not been established before—those with Primary Progressive MS (PPMS). Unlike other forms, PPMS does not present relapses; instead, neurological symptoms gradually worsen over the years, often leading to irreversible disability.
Historically, only a few medications have shown potential benefits in PPMS, with Ocrelizumab being one of them. Previous studies indicated its ability to slow disability progression but primarily focused on younger patients.
Benefits for Advanced Disease
What makes this recent study significant is its inclusion of older patients, with a maximum age of 65, as well as those who are heavily reliant on wheelchairs. The results demonstrated the effectiveness of Ocrelizumab even among severely affected individuals. The study involved approximately 1,000 patients, half of whom received Ocrelizumab infusions every six months, while the other half were given a placebo.
The findings were striking: disability progression occurred in 40% of the placebo group compared to 33% of the Ocrelizumab group. Statistically speaking, this equates to a 30% reduction in the risk of progression.
Why Has This Been Unknown Until Now?
The question arises: Why has the effectiveness of Ocrelizumab in advanced PPMS only been demonstrated recently? This knowledge gap is partly attributed to the parameters used to measure drug efficacy. In most MS studies, the Expanded Disability Status Scale (EDSS) serves as the endpoint. While widely used, the EDSS primarily reflects mobility, leaving out other critical aspects.
For patients who are already highly disabled, even a marginal decline in their abilities might not significantly affect their EDSS score. Hand function, essential for performing daily tasks such as feeding, dressing, and writing, often receives inadequate attention in this scale.
Hand Function: A Vital Concern
The new study rectified this oversight by evaluating hand function alongside the EDSS. Appropriately titled the ORATORIO-Hand study, it combined the EDSS assessment with the Nine-Hole Peg Test, which measures the time it takes to place small pegs into holes. A significant increase in completion time over the study duration—which indicated worsening—occurred less frequently in the Ocrelizumab group compared to the placebo group.
The study did not discover a new mechanism of action or introduce a new drug; rather, it opened a door to understanding how Ocrelizumab can effectively influence disease progression even in older patients with substantial disabilities.
A Paradigm Shift for MS Care
The results of this study could potentially alter the clinical approach to MS treatment. It emphasizes that even in advanced stages, the course of the disease can still be influenced. Impaired mobility does not equal a “burned-out” disease that cannot be impacted. Especially in these cases, maintaining hand function becomes crucial for a patient’s independence and autonomy.
